2. Mail or drop off the completed form to:

due at time of pickup. Materials will not be provided without prior payment.

Some records may be exempt from disclosure as outlined in ORS 192. Records that are released may contain redactions.
These may include medical information, trade secrets, statements that would constitute an invasion of privacy, etc.
